Osteochondritis Dissecans Associated with Lateral Discoid Meniscus of the Knee




Abstract
The knee is recongnized as the most common site of osteochondritis dissecans which are most often affected in the laereal aspect of the medial femoral condyles. Although the cause of osteochondritis dissecans is unknown, repetitive trauma or
circulatory defect leading to osteonecrosis of subchondral bonc have been postulated as the major causative factor. From September 1983, the authors have experienced, even though its incidence is low, 7 cases / 6 patients of osteochondritis dissecans in the
lateral femoral condyle associated with lateral discoid meniscus and discussed the pathomechanism and the clinical significance of them through over 1 year follow-up period. Arthroscopic removal and trimming of crater were performed in 5 cases, K-wire
fixation
in 2 cases, and all the 7 cases were done arthroscopic reshaping of the discoid meniscus. As a result, we rated 5 cases as good, 2 cases as fair according to rating criteria of Hugthston. It might be suggest to reshape the thickend discoid
meniscus
as
early as possilbe to protect the femoral condyle from abnormal shear stress.
